Hazel Lee Clark, 78, passed away September 7, 2015, surrounded by her family at Rainey Hospice House.
She was a member of Gethsemane Baptist Temple.
Hazel is survived by her daughters; Connie Thrasher, Debbie Bannister and Cecilia Dale Donald; seven grandchildren; seventeen great grandchildren and her beloved cat Tom.
Hazel was predeceased by her husband Cecil Clark.
The family would also like to extend their sincerest thanks and gratitude to her doctors and the entire staff on the 7th floor at AnMed Health. Their dedication, professionalism, and caring services were above and beyond the normal call of health care.
Services will be held at Sosebee Mortuary, South Chapel on September 11, 2015 at 2:00 pm, officiated by Rev. Sam Duncan.
